What moved in longevity science this week, what it changes for what gets built, funded, approved and priced, and what is being sold far ahead of its evidence. Global signal, India lens.
This week in 30 seconds
| Not yet | Regeneron says trevogrumab preserved lean and muscle mass during GLP-1 weight loss. Funders now have a named Phase 2 asset in the muscle-preservation class. With no effect size, no peer-reviewed report and no strength or fracture outcome released, there is nothing yet to price against, and prescribers get nothing. |
| Not yet | Longevity clinic leaders called for standardised care as health systems open their own programmes. Health systems and clinic operators are building on this; patients paying up to $250,000 a year still buy a panel with no outcome data behind it. |
| Contested | Is epigenetic age acceleration measuring aging, or the disease the patient already has? One clock rose with tumour stage, while two other papers used clocks as aging measures in disease populations. |

Not yetFunders now have a named Phase 2 asset in the muscle-preservation class. With no effect size, no peer-reviewed report and no strength or fracture outcome released, there is nothing yet to price against, and prescribers get nothing.
| Who | Regeneron, with BodySpec selling the measurement |
|---|---|
| What | A Phase 2 topline (COURAGE) and a retail scan launch |
| How much | No trial numbers released; scans under $60, about 12 minutes |
| Evidence | Stage not assignable: a company topline carried by press coverage, and a retail scan launch |
| Product, price | BodySpec DEXA body-composition scan, under $60; trevogrumab has no approved product or price |
| under $60scan price | nonefunctional endpoints reported |
◆ Biomarkers are not outcomesLean mass stands in for strength, walking speed and fracture risk, none reported.
Regeneron reported Phase 2 COURAGE topline results for trevogrumab, an antibody against the myostatin pathway. The company says it preserved lean and muscle mass during GLP-1-induced weight loss, without meaningfully increasing total weight loss. No effect size was released, and the result reached us through press coverage.
The adjunct is now a named Phase 2 asset rather than a thesis, and BodySpec is selling the matching measurement: DEXA body-composition scans from an Oakland storefront, aimed at GLP-1 users worried about muscle.
The indication's yardstick is unstable: repeating a grip-strength measurement changes which patients count as low-strength under the 2025 Asian Working Group for Sarcopenia criteria. No study design is in the available reporting, so no evidence stage is assignable. 1–3
HARMSNo adverse-event data accompany the topline, and no source reports whether preserved mass prevented a fall. No harms, outcome or interpretation data accompany the retail scan either: a lean-mass number does not tell a buyer whether strength or function changed.
WHAT WOULD MOVE THISA peer-reviewed COURAGE report carrying strength, gait-speed or fracture outcomes alongside the mass data.

| Who | New York Times reporting, a Roundtable of Longevity Clinics discussion, CNBC |
|---|---|
| What | Market structure: health-system programmes, a standardisation push, concierge pricing |
| How much | Up to $250,000 a year per patient, per CNBC |
| Evidence | Stage not assignable: press reports, a conference discussion and a commentary carry no study design |
| Product, price | Concierge longevity care, up to $250,000 a year; no other price list appears |
| up to $250,000 a yearconcierge spend |
The New York Times reported that longevity medicine is gaining credibility as major health systems open their own programmes. At the fourth Roundtable of Longevity Clinics, chaired by Buck Institute CEO Eric Verdin, clinic leaders argued for moving from luxury diagnostics toward standardised care.
A sector asking for standards is telling you its current offer is not standard. CNBC reported some wealthy Americans spending up to $250,000 a year on concierge care, while a commentary argues hospital longevity clinics monetise time and attention rather than proven medicine.
Distribution widened, not prediction: none of the four sources carries outcome data, and a teaching-hospital logo does not make a panel predict more. 4–7
HARMSNone of the four sources reports harms, testing cascades or the cost of chasing incidental findings.
WHAT WOULD MOVE THISA published outcome dataset from one of these programmes: who was tested, what changed, at what cost per patient.

Is epigenetic age acceleration measuring aging, or the disease the patient already has?
| Reading A | It is tracking disease burden. PhenoAge acceleration rose with tumour stage in 3,067 patients undergoing primary tumour resection (People, watched, observational cohort biomarker clock construct-validity analysis, preprint). The authors conclude it may track cancer burden rather than biological aging, which would make that readout hard to interpret in a patient with active disease, on one unreplicated preprint. |
|---|---|
| Reading B | It is tracking aging, and disease is how aging shows up. Epigenetic age acceleration tracked diabetic kidney disease progression in an observational cohort comparing 10 epigenetic clocks across two cohorts, n=1259 (People, watched). Pretreatment blood DNA methylation profiling was associated with survival on immune checkpoint inhibitors in a retrospective observational cohort of 100 head and neck cancer patients (People, watched). If aging drives disease, these correlations are expected. |
| What would settle it | A clock measured before disease onset that predicts incident disease, or a clock moved in a trial with a clinical endpoint. Neither appears in this week's record. 8–10 |

| Partial epigenetic reprogramming toward the clinic | ↑ rising◆◆◆◇◇ People, watched | ER-100's first-in-human readout recirculated from W41 with no new data (14132, People, tested on a stand-in, Phase I, single-arm, company-reported). The week's new work was preclinical: lipid-nanoparticle delivery of circular RNA to senescent cells, and cyclic OSKM reducing 48-hour brain lesion area in aged mice, not a recovery measure. |
| Epigenetic and proteomic age clocks, commercialisation | ↑ rising Measurement | A new vendor entered and a clock trial was registered; no clock in this week's record was validated against a clinical outcome under intervention, and PhenoAge acceleration rose with tumour stage. |
